This Privacy Policy explains how Liz Patton & Associates collects, uses, stores, and protects your personal and health information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).
What we collect We collect personal details (name, date of birth, address, contact information), health information (presenting concerns, medical history, medications, mental health history), Medicare and health fund details, NDIS plan details, GP and referrer details, emergency contact information, and session notes.
How we collect it Information is collected via our patient registration form, during clinical sessions, and through correspondence with referrers and treating practitioners (with your consent).
Why we collect it To provide safe and effective psychological services, process Medicare and NDIS claims, meet mandatory reporting obligations, and enable professional supervision and quality assurance.
Storage and security All records are stored securely in Halaxy practice management software using industry-standard encryption. Records are retained for a minimum of seven years from the date of last service (or until a minor turns 25, whichever is longer).
Your rights You have the right to request access to your clinical records. Please contact us at contact@lizpatton.com.au. You may also request corrections to your records at any time.
Data breaches In the event of a data breach, we will comply with the Notifiable Data Breaches scheme and notify affected individuals and the Office of the Australian Information Commissioner (OAIC) as required.
